UYENO FOUNDATION GRANT APPLICATION

The Uyeno Foundation provides grant funding to support the efforts of local nonprofits serving the needs of people with mental health challenges in the Southwest Florida area. As a nonprofit 501©3, The Uyeno Foundation is committed to removing the stigma associated with mental health.

We believe that by funding initiatives that promote positive mental health through prevention, intervention, treatment, and education, we can attain our vision of a community without barriers to access for mental health support services, regardless of income or ability to pay.

This grant opportunity is available only to organizations that are providing mental health support services. The Uyeno Foundation is currently offering grant opportunities of up to $1,000 to organizations that are providing mental health support services. Organizations must be focused on serving individuals and families with prevention, intervention, treatment or education for mental health related illnesses. The purpose of this grant is to assist qualified 501©3 nonprofit organizations accomplish the work they are doing in the community.

The Uyeno Foundation reserves the right to contribute additional funds above the grant amount to recipients when funds are available at their sole discretion.

ELIGIBILITY

  • Must be a not for profit 501©3 or other eligible not for profit organization
  • Agency must provide mental health services
  • Agency must provide direct services to clients
  • Agency must have a non-discrimination policy
  • The Uyeno Foundation reserves the right to deny applicants which have previously received grant funds from the Uyeno Foundation and have not complied with the terms of the grant contract

REVIEW & SELECTION PROCESS

Each proposal will be reviewed by The Uyeno Foundation Board. No additional information will be accepted after the stated proposal deadline of June 30, 2021.  The written proposal and supporting documentation will be the sole basis for review.  Awards will be announced no later than August 1, 2021.  Upon receipt of an award, the recipient will be offered a contract with The Uyeno Foundation, Inc. that outlines reporting requirements.

REPORTING REQUIREMENTS

Recipients will be required to file a report at the end of one year. Funds not documented must be returned. Grant recipients will be required to share a detailed success story and may be invited to share the impact at an event recognizing their success.

Applications MUST be submitted to the Uyeno Foundation office to arrive no later than 5 PM on June 30, 2021. The Uyeno Foundation will not notify applicants of receipt of applications. PLEASE NOTE: A Year End Report is required of each grant recipient at the end of the year of receipt, specifying how grant monies were used.
